Don’t forget your documents
To complete your medical records, please bring an identity document with you.
If the patient is over 18:
- a passport (a photo of the document is acceptable).
If the patient is under 18:
- the passport of a parent or legal guardian (a photo of the document is acceptable);
- the child’s birth certificate (a photo of the document is acceptable).
The presence of a parent or legal guardian is required. If the child is accompanied by someone else, a notarised power of attorney from the legal guardian must be provided.
Please arrive at the clinic 15 minutes before your appointment so we can complete the necessary paperwork and sign the agreement.
For your convenience, you can review the medical services agreement in advance: Medical services agreement
Allow time at your first visit to sign the contract
On the first visit, a contract for the paid provision of dental services is concluded with the Patient or their legal representative. The Patient signs an informed voluntary consent to medical intervention and a consent to the processing of personal data, and an outpatient record is created.
The Patient should arrive at the clinic 15 minutes before the appointment.
If the contract and/or the informed voluntary consent to medical intervention is refused, the clinic has the right to refuse the appointment.
If you have recent test results — bring them with you
To establish, rule out or confirm a diagnosis, or to prescribe treatment or diagnostics, additional examinations prescribed by the doctor may be required. If you have current test results obtained at other medical organisations, you are entitled to provide them to our doctor at the appointment.
